need to cancel your appointment.


After Hours Emergencies
• Please go to the nearest emergency room for
after hours emergencies or call 911

What to Bring to the First Visit
• Please download, print and complete the “New Patient Registration Packet” and bring this to your first visit
• Insurance Card
• Drivers license or valid photo identification
• Payment for any applicable co-pays
• Any relevant MRI/CT/X-ray or other studies
including lab work
I’ve been scheduled for an interventional pain procedure…now what?
• Download and print the Pre-Operative
Instructions. These instructions are self
explanatory.

I’ve had an interventional pain procedure with a CPM physician…now what?
• You will be given post-operative instructions following your procedure(s). Read them and follow them carefully.
• Please make a follow-up office appointment for 2 weeks after your LAST procedure (sometimes procedures are booked in aseries) by calling (210) 805-9800 or toll free (800) 422-3203.

Payment Policy
• Co-pays are due at the time service is
rendered.
• As a courtesy your insurance will be billed for additional charges.

Please be advised that Consultants in Pain Medicine is in compliance with all HIPAA guidelines. The following is a summary of our HIPAA Notice of Privacy Practices:


This notice describes how medical information about you may be used and disclosed and how
you can get access to this information. Please review it carefully.

Our Office uses health information about you for treatment, to obtain payment for treatment, for
administrative purposes, and to evaluate the quality of care that you receive. Your health information is contained in a medical record that is the physical property of Consultants in Pain Medicine.

This Notice of Privacy Practices describes how we may use and disclose your protected health
information to carry out treatment, payment or health care operations and for other purposes that are
permitted or required by law. It also describes your rights to access and control your protected health
information. “Protected health information” is information about you, including demographic information, that may identify you and that relates to your past, present or future physical or mental health or condition and related health care services.

We are required to abide by the terms of this Notice of Privacy Practices. We may change the terms of
our notice, at any time. The new notice will be effective for all protected health information that we
maintain at that time. Upon your request, we will provide you with any revised Notice of Privacy Practices by accessing our website at www.cipm.com, calling the office and requesting that a revised copy be sent to you in the mail, or asking for one at the time of your next appointment.
